About the Role

This position is based with the Acute & Continuing Care Team (ACCT), a multidisciplinary team responsible for providing acute response, assessment, planning, and case management for adults living in the community who are experiencing an acute episode and who may require continuing care and case management.

The team provides mental health support for adults who are experiencing a mental illness, utilizing a recovery-oriented approach that is person-centered, trauma-informed, and involves partnering with general health providers and agencies that can offer clients community living supports such as housing, disability services, vocational, social, and educational opportunities.

Key Responsibilities:

  • Provide acute response, assessment, planning, and case management for adults living in the community who are experiencing an acute episode.
  • Collaborate with the Acute & Continuing Care Team to provide mental health support for adults who are experiencing a mental illness.
  • Develop and implement individualized care plans that address the unique needs and goals of each client.
  • Partner with general health providers and agencies to provide community living supports for clients.
  • Contribute to the development and implementation of team policies and procedures.

Requirements:

  • Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Experience working in acute care and community mental health settings.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Knowledge of recovery-oriented approaches and trauma-informed care.
