About the RoleWe are seeking a highly motivated and skilled Mental Health Care Navigator to join our team at NSW Health.
You will take a child-centred approach, focusing on the needs of the child and their family. You will engage and support children and families within the Hub, guiding them through the support process and coordinating with other service providers to ensure continuity of care.
This is an opportunity for you to contribute to delivering high-quality clinical services to children experiencing mild to moderate mental health and well-being challenges.
Your Key Responsibilities- Act as the first point of contact for clients, determining their needs using assessment and problem-solving skills.
- Engage and support children and families within the Hub, ensuring seamless collaboration with other service providers.
- Collaborate with internal and external care providers across health, education, social services, Aboriginal health, and disability sectors.
- Work closely with experienced multidisciplinary teams to deliver exceptional clinical services.
