We are seeking a highly skilled and compassionate Practice Nurse to join our team at Hope Community Services. As a key member of our multidisciplinary team, you will play a vital role in supporting the physical and sexual health needs of young people aged 12-25 years.
Key Responsibilities- Provide person-centered support, education, referral, and advocacy to young people regarding their physical, sexual, mental, and psychosocial health.
- Work closely with key stakeholders and the Community Engagement team to promote health and wellbeing in the community and within secondary schools.
- Build capacity within the multidisciplinary team by providing secondary consultation.
- Deliver high-quality physical and mental health assessments with young people.
- Develop and implement health education programs and interventions to promote healthy lifestyles and prevent health problems.
- Current Registration as Registered Nurse (Division 2) with AHPRA with tertiary qualifications in Nursing (RN Yr 2 +).
- Registered Nurse with current AHPRA (Australian Health Practitioner Regulation Agency) registration.
- Certificate of sexual Health /or sexual health training essential.
- Experience working with young people plus demonstrated knowledge of and sensitivity to the mental health needs of young people (12 – 25 years) and their families and friends.
- Minimum 2 years post graduate nursing experience working in a mental health area.
