About the Role

We are seeking a skilled Health Care Worker to join our Specialist Dementia Care Program team. As a key member of our team, you will work collaboratively with other healthcare professionals to deliver high-quality care services to individuals with dementia and their families.

Your Key Responsibilities

  1. Deliver person-centred care to individuals with dementia, focusing on their physical, emotional, and social needs.
  2. Work closely with the multidisciplinary team to develop and implement individualised care plans that promote dignity, respect, and choice.
  3. Provide support and education to family members and carers to empower them to provide effective care and make informed decisions.
  4. Contribute to the development and implementation of policies and procedures that promote safe and effective care practices.
  5. Participate in quality improvement initiatives to enhance the quality of care and services provided by Bendigo Health.

Your Skills and Qualifications

  • Degree in a relevant field (e.g. nursing, allied health, social work)
  • Experience working in aged care or disability services
  • Strong communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
  • Ability to maintain confidentiality and handle sensitive information
