About the Role

We are seeking a highly skilled and dedicated Dietitian to join our team as a Mobile Care Specialist. As a key member of our healthcare team, you will provide high-quality dietetics services to adults living in Residential Aged Care facilities and the local community.

Key Responsibilities
  • Provide person-centered dietetics care to all clients, ensuring their nutritional needs are met and exceeded.
  • Collaborate with healthcare teams to improve nutrition and hydration systems in residential aged care homes, promoting a culture of excellence and best practice.
  • Develop and deliver educational sessions to clients, caregivers, and healthcare professionals, enhancing their knowledge and skills in nutrition and dietetics.
  • Conduct menu and mealtime assessments, providing expert recommendations to optimize client outcomes.
  • Work closely with external stakeholders and Plena Healthcare colleagues to ensure seamless delivery of high-quality dietetic care.
  • Embody a commitment to continuous improvement, staying up-to-date with contemporary best practices and industry developments.
  • Maintain accurate and timely clinical documentation, ensuring seamless communication with healthcare teams.
  • Contribute to a positive team culture, upholding Plena Healthcare's values and actively engaging with dietetics colleagues.
Requirements
  • Bachelor's or Master's Degree in Dietetics from an accredited institution.
  • Current registration with the Dietitians Association of Australia (DA).
  • Ability to travel extensively, with a willingness to adapt to changing schedules and priorities.
  • NDIS workers screening and National Police Check, as required.
  • Australian working rights and proof of Covid-19 and current flu vaccination.
