Dementia Care Manager

HammondCare Horsley is seeking a dedicated and compassionate Manager to support our complex behaviour cottages for people living with dementia.
As a key member of the Horsley management team, you will work closely with various stakeholders, including the Residential Care Operations team, Local Health District, SDAs (RNs), SDCs (Carers), residents, and the multi-disciplinary health team to ensure resident outcomes are met.
About the role:
As the Manager for complex behaviour cottages supporting people living with dementia, you will be responsible for:
- Advocating for residents to ensure their care is person-centred and focused on their strengths, capacities, and promotes their well-being at every stage.
- Facilitating clinical care delivery by the SDAs (Registered Nurses) and SDCs (Carers).
- Providing hands-on modelling, coaching, and mentoring in clinical and complex behaviour management areas and processes.
- Consulting in case management and clinical decision making.
- Providing clinical oversight and professional competence as a Registered Nurse with experience in complex dementia-specific care.
- Supporting and investigating clinical/resident incidents as required.
- Communicating effectively with all stakeholders, both within HammondCare and external partners to achieve the best outcomes for the residents.
Requirements:
- Tertiary qualifications in specialised dementia and/or mental health service nursing for older people.
- Significant experience operating as a consultant or specialist nurse preferably within Aged Care.
- Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team, and effectively work with other staff.
- Strong leadership and influencing skills.
- Attitude of service.
- Strong negotiation and conflict resolution skills.
- Excellent analytical skills.
- Strong administrative and computer skills.
- Ability to work independently and as part of a team.
- Empathy and patience.
