Job Title: Compassionate Caregiver
\We provide personalized nursing care to seniors and adults with disabilities in their homes, empowering them to maintain their independence and quality of life.
\Our mission is to deliver exceptional client-focused care in a supportive community setting. To achieve this, we require nurses who can provide comprehensive care, assess client needs, and develop tailored care plans in collaboration with our multidisciplinary teams.
\Key Responsibilities:
\- Provide holistic nursing care to clients in their residences\
- Conduct thorough assessments and create customized care strategies for each client\
- Collaborate with interprofessional teams to ensure optimal client outcomes\
- Perform regular evaluations to guarantee client safety and well-being
Requirements:
\- Current registration with the relevant health authority\
- Australian National Police check and Working With Children's Check\
- Current first aid and CPR certification\
- Driver's license and reliable vehicle
PREFERRED SKILLS:
\- Experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS support coordinator\
- Intermediate to advanced proficiency in Microsoft Office applications (Excel, Word, Outlook, and CMS)
