Community Mental Health Specialist – Recovery-Focused Programs

About the Role

This is a challenging and rewarding role for a qualified Community Mental Health Specialist to work with clients who require intensive support and case management. The successful candidate will oversee a caseload of clients discharged from Thomas Embling Hospital, providing linkage support to area mental health services and conducting comprehensive assessments and interventions.

Key Responsibilities:

  • Providing high-quality support for treatment-ordered clients post-discharge and case management of clients under Custodial Supervision Orders (CSO) on Extended Leave.
  • Conducting risk assessments, crisis plans, and care development for high-risk clients in the community.
  • Preparing case-managed clients for court and engaging in relevant legal processes under various legislation.
  • Participating in intra-hospital transfer or discharge planning of patients from TEH.
  • Supporting carers regarding ongoing treatment and care.

Requirements:

  • Demonstrated commitment to mental health consumer recovery principles.
  • A minimum of 5 years post-qualification experience in mental health, preferably recent community mental health experience.
  • Approved degree in Social Work, Occupational Therapy, Psychiatric Nursing, or Psychology, registered with APHRA.
  • Knowledge of community resources, including drug and alcohol services and accommodation services.
  • Understanding of relevant legislation, mental health systems, and the criminal justice system.
  • Proven experience in conducting mental health and risk assessments.
