- Key Responsibilities:
- Deliver personalized case management that caters to each client's unique needs, objectives, and preferences.
- Work collaboratively with clients to create and review individualized budgets that ensure alignment with their care goals.
- Manage a caseload, including regular client assessments and reviews to guarantee quality, holistic care.
- Establish and maintain strong, trusting relationships with clients and their families.
- Coordinate services across allied health, nursing, home support, and equipment providers to ensure seamless delivery.
About the Position:
- We seek an experienced and motivated professional to join our Home Care team as a Coordinator.
- You will bring a proven track record in aged care case management (HCP/CHSP preferred), minimum 2 years' experience in community or aged care coordination, and exceptional planning, coordination, and client advocacy skills—especially within reform environments.
- Nursing qualifications (RN or EN) are highly regarded.
- Excellent communication, interpersonal, and problem-solving abilities are essential.
- Candidates should possess the capacity to work independently while contributing to a multidisciplinary team.
- A passion for delivering high-quality care with meaningful outcomes is required.
- Eligibility for a National Police Check and NDIS Worker Screening Check (or willingness to obtain) is necessary.
- A valid Australian driver's licence and clean driving history are also required.

- Ability to demonstrate flexibility and adaptability when interacting with diverse clients and stakeholders.
- Maintain accurate and detailed records of client interactions and care plans.
- Collaborate effectively with healthcare professionals to develop and implement comprehensive care plans.
- Cultivate partnerships with local businesses and community organizations to enhance service delivery.
- Contribute to the development and implementation of policies and procedures to improve service quality.
