We are seeking a highly skilled and experienced Registered Nurse to join our team as a Nurse Unit Manager at St Vincent's Hospital Melbourne. As a key member of our interdisciplinary team, you will be responsible for leading and managing the daily operations of our Bed Based Transition Care Program.
Key Responsibilities- Lead and work with a collaborative, supportive, and highly functioning interdisciplinary team to provide exceptional holistic care to older persons.
- Develop and implement clinical nursing practices that promote patient-centered care and improve health outcomes.
- Manage the daily operations of the Bed Based Transition Care Program, ensuring seamless transitions for patients and families.
- Collaborate with other healthcare professionals to develop and implement care plans that meet the unique needs of our patient cohort.
- Current Nursing Registration with AHPRA.
- Sound knowledge of the Aged Care sector and awareness of the policy environment underpinning the Transition Care Program.
- Demonstrated ability to lead, collaborate, and engage with all members of the interdisciplinary team.
- Previous management or leadership experience in a healthcare setting.
- A culture of continuous improvement, where your contributions make a difference.
- A supportive senior leadership team.
