The HOPE Suicide Prevention Program, in partnership with Monash Health Dandenong, aims to revolutionise the way we address suicidal behaviours in the Dandenong region. This pioneering initiative focuses on empowering individuals to connect with supportive networks, providing guidance, encouragement, and follow-up services to ultimately improve response, treatment, and support for those at risk.

The Role

The Psychosocial Support Coordinator will play a vital role in delivering the HOPE Suicide Prevention Program, providing non-clinical assertive outreach support to individuals who have attempted suicide or are experiencing a suicidal crisis. This role will involve case management, developing safety and well-being plans, and facilitating linkages with services and life-sustaining supports.



This position is part of a multidisciplinary team working in a collegiate and learning environment, with quality supervision and clinical governance systems in place.



Key Responsibilities


  1. Provide non-clinical assertive outreach follow-up support to individuals who have attempted suicide
  2. Develop and implement safety and well-being plans to ensure the individual's safety and reduce the likelihood of a further attempt
  3. Provide support and follow-up with family members, friends, and support networks to facilitate continued support and recovery
  4. Facilitate linkages with services and life-sustaining supports

Requirements

To be considered for this role, you will need:




  1. Qualifications in a relevant discipline, such as Social Work, Case Management, or Community Welfare
  2. Demonstrated experience in a similar role providing non-clinical outreach support to vulnerable clients
  3. Demonstrated case management skills and ability to work effectively with clients, family members, and service providers
  4. Sound understanding of the personal, family, and social issues associated with suicidal ideation and attempts


In addition, you will need to have a proven ability to work with diverse communities, including Aboriginal and Torres Strait Islanders, LGBTI individuals, older people, young people, and those experiencing mental health conditions.
