Occupational Therapist

Alfred Health is seeking a skilled Occupational Therapist to join our Adult Health team at Caulfield Community Health Service.
About the RoleThe successful candidate will provide comprehensive assessments, recommendations, and ongoing OT services to clients living at home. They will develop and implement person-centered care plans, work within a multi-disciplinary team, and liaise with relevant agencies to coordinate and facilitate client treatment and outcomes.
Key Responsibilities- Provide occupational therapy services to clients living at home, including assessments, recommendations, and ongoing support.
- Develop and implement person-centered care plans that meet the unique needs of each client.
- Work collaboratively with a multi-disciplinary team to ensure a holistic approach to client support.
- Liaise with relevant agencies, including local councils and contractors, to facilitate installation of home modifications and equipment prescription.
- Demonstrate an understanding of pathways within the healthcare system to advocate for clients with complex care needs.
- Degree in Occupational Therapy.
- Registration with the Occupational Therapy Board of Australia and adherence to continuing professional development requirements.
- Registration with the State-wide Equipment Program (SWEP).
- At least two years of experience in occupational therapy, including demonstrated clinical experience relevant to the community health sector.
- Experience in home modifications and equipment prescription.
- Current Victorian driver's license.
