Child and Family Health Professional

We are seeking a passionate and experienced professional to join our team in providing quality child and family health services. As a key member of our team, you will work with families caring for children 0-3 years of age, using a strength-based and wellness-focused approach to build parental confidence and skills.
This is an exciting opportunity to be part of our innovative service delivery model. You will work as part of a team with an Aboriginal Health Worker, establishing connections and opportunities for collaboration with local service providers to improve pathways to care and support for families.
Key Responsibilities:
- Provide quality child and family health services to families caring for children 0-3 years of age
- Work within the Tresillian Model of Care, Practice Principles and the NSW Health Professional Practice Framework for Child and Family Health (CFH)
- Use evidenced-based screening and assessment tools to identify health variations or family vulnerabilities
- Negotiate individual care plans and discharge pathways to meet the goals of the family
Qualifications and Skills:
- A current registration as a Registered Nurse and/or Registered Midwife
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
