**Job Title:** Senior Case Manager

Overview:

We are seeking a skilled and compassionate Senior Case Manager to join our team. As a Senior Case Manager, you will play a critical role in helping individuals with intellectual and/or developmental disabilities identify and achieve their long-term and short-term goals. You will work closely with our members to develop, implement, and monitor personalized Life Plans that cater to their unique needs and aspirations.

About the Role:

This dynamic position involves advocating for and with our members to ensure informed decision making, informed consent, and appropriate guardianship. You will be responsible for scheduling, leading, and collaborating with our members and their interdisciplinary teams to conduct meetings and assessments. This will enable the development of comprehensive, person-centered Life Plans that reflect the individual's needs and desired life goals.

Responsibilities:

  • Advocacy and Support: Advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
  • Life Plan Development: Schedule, lead, and collaborate with our members and their interdisciplinary teams to conduct meetings and assessments, enabling the development of comprehensive, person-centered Life Plans.
  • Implementation and Monitoring: Implement, update, and monitor Life Plans, facilitating individualized Life Plan reviews and approval processes.
  • Integration of Supports: Ensure integration of all needed and preferred supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, community-based, and facility-based long-term supports and services.
  • Ongoing Contact: Maintain ongoing contact with critical people in a member's life.
  • Documentation: Ensure timely submission of all documentation (Life Plan, Progress notes, etc.).
  • Benefits Assistance: Assist our members with maintaining benefits such as Social Security, Supplemental Security Income, Medicaid, and Medicare coverage, and Food Stamps.
  • Problem Resolution: Assist members to resolve problems in living, such as housing, utilities, the judicial system, and general safety.
  • Reporting Abuse: Report abuse or neglect immediately when observed or reported.

Qualifications:

  • A Bachelor's degree with 2+ years of relevant experience, OR
  • A License as a Registered Nurse with 2+ years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
  • A Master's degree with 1+ year of relevant experience.

Requirements:

  • Strong communication skills, including verbal and written communication skills, along with strong interpersonal and organizational skills also required.

Salary:

The salary for this position starts at $27.75 per hour and can go up to $30.19 per hour.
