Care Coordinator
Are you a skilled clinician looking to transition into a care coordination role?
This exciting opportunity allows you to utilize your clinical expertise to ensure comprehensive, client-centered management plans are achieved for our clients. You will conduct initial phone assessments, care planning, and goal setting, coordinate in-home services, build relationships with stakeholders, monitor patient progress, and provide telephone support, education, and advice.
We offer flexible working options, including a rotating 7-day roster between the hours of 7am & 8pm. Alternate weekends are worked from the comfort of your own home. This role is based in Adelaide (South Australia), offering a great work-life balance.
Key Responsibilities:
- Conduct initial phone assessments, care planning, and goal setting
- Coordinate in-home services to support clients receiving care at home
- Build and maintain effective relationships with a variety of stakeholders
- Monitor patient progress and update care plans as appropriate
- Liaise with healthcare professionals and specialists
- Discharge planning, including referrals to long-term services
Requirements:
- 3-5 years' experience in an acute hospital setting as a Registered Nurse with unrestricted registration
- Recent experience in care coordination (highly desirable)
- Strong customer service orientation and relationship management skills
- Excellent clinical reasoning skills
- Ability to work in a fast-paced environment
- Proficient computer skills to enable fast and accurate data entry into patient electronic medical records
