About the Role

We are seeking a skilled healthcare professional to contribute to the wellbeing of individuals with physical, developmental, or cognitive disabilities in our Supported Disability Accommodation.

As a key member of our team, you will be responsible for conducting thorough assessments, care planning, direct nursing care, and reviewing cares using the biopsychosocial model to optimise customers' independence.

You will also provide workplace coaching, guidance, and supervision to personnel Carers, Assistant in Nursing, and Enrolled Nurses, and act as a role model for practice and professional conduct.

Furthermore, you will ensure all Workplace Health and Safety, infection control, and safe work practices are always followed.

You will provide written and verbal reports on the social, physical, and emotional well-being of residents to the Clinical Nurses/Assistant Manager and collaborate with them to achieve quality outcomes for residents.

Key Responsibilities:
  • Conduct thorough assessment, care planning, direct nursing care, and review cares using biopsychosocial model
  • Provide workplace coaching, guidance, and supervision to personnel
  • Ensure all Workplace Health and Safety, infection control, and safe work practices are always followed
  • Provide written and verbal reports on the social, physical, and emotional well-being of residents
