As a key member of our multi-disciplinary team, you will be responsible for facilitating the delivery of care to our valued senior customers. Key Responsibilities- Facilitate clinical management and coordination efforts in residential aged care facilities across Victoria.
- Orchestrate seamless rounds with Geriatricians, Physicians, and GPs, ensuring clear communication between on-site staff and remote healthcare providers.
- Collaborate with a multi-disciplinary team to ensure efficient and high-quality service delivery.
- Contribute to our care model by engaging in organic business development to expand the resident base.
- Bachelor's or Diploma of Nursing.
- Experience in problem-solving and professional collaboration.
- IT skills and proficiency in documenting customer needs.
- AHPRA Nursing Registration.
- Aged Care experience, leadership preferred.
- Current driver's license and reliable vehicle.
