Dementia Care Professional

A rewarding opportunity for passionate Dementia professionals to join HammondCare on a Fixed Term Full-time contract. This role is based in Hobart Office.
As a Dementia Support Coach, you will facilitate on-the-ground support, implement relationship-based strategies, model and coach clients, carers and care staff.
- Work collaboratively with peers to develop skills, expertise and knowledge of aged care staff and carers to understand individuals living with dementia and provide best practice support and care.
- Support the implementation of recommendation plans, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
- Facilitate and support trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
- Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
- Ability to work both independently and as part of a team.
- Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure consistent provision of exceptional service to internal and external customers.
- Current driver's licence and safe driving record.
- Willingness to travel and work intrastate and interstate for up to five days at a time to support delivery of services.
- Flexibility to work flexible hours at times and as required to meet assessed needs of clients and carers.
- Qualification in relevant field, such as Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable.
- Estimated salary: $70,000 - $90,000 per year, depending on experience.
