Overview of the Position:

We are seeking a dynamic and enthusiastic individual to fill the position of Long Stay Patient Care Specialist.

The successful applicant will provide high-level strategic and clinical leadership across East Metropolitan Health Service regarding long-stay patient management. This role involves providing expert advice, consultation, and authoritative counsel to all EMHS staff around long-stay patient management issues, including NDIS and Aged Care, especially where highly complex physical and psychosocial needs are identified.

Facilitating clinical systemic problem-solving and escalations is also an essential aspect of this position. The successful applicant will work closely with EMHS staff to develop and implement effective strategies for managing long-stay patients, ensuring that their needs are met in a timely and efficient manner.

Key Responsibilities:

  • Providing strategic and clinical leadership in long-stay patient management
  • Developing and implementing effective strategies for managing long-stay patients
  • Facilitating clinical systemic problem-solving and escalations
  • Collaborating with EMHS staff to ensure the needs of long-stay patients are met

Required Skills and Qualifications:

  • Masters or PhD in Nursing, Medicine, or related field
  • Minimum 5 years of experience in healthcare management, preferably in long-stay patient care
  • Strong leadership and communication skills
  • Ability to analyze complex data and develop effective solutions
