Post Admission Support Team Member

Job Summary: We're seeking a skilled clinician to facilitate seamless transitions for clients from acute mental health inpatient settings to the community, ensuring safe, coordinated, and streamlined care.
About the Role: As a PAST Clinician, you will provide expert clinical input to maximize connections with community supports, minimize early readmissions, and build strong partnerships with clinicians within and outside of Mercy Health.
Key Responsibilities:
- Develop and implement effective transition plans to ensure clients receive comprehensive care in the community.
- Collaborate with multidisciplinary teams to identify and address client needs, promoting timely and efficient transitions.
- Provide clinical guidance and support to clients, families, and carers to ensure a smooth transition to community-based care.
Qualifications:
- A relevant degree in Psychology, Occupational Therapy, or Social Work, with registration and/or membership with relevant professional bodies.
- Excellent clinical, communication, and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- A compassionate and flexible approach to patient care, with a commitment to delivering high-quality services.
