Responsibilities:

  • Deliver person-centered care and support services to clients in their homes or in residential settings
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans

Qualifications and Experience:

  • Minimum 6 months experience in aged care or a related field

  • Fully vaccinated as required by state government regulations
  • Ability to obtain NDIS worker screening card
