About the Role

We are seeking a skilled Home and Community Care Specialist to join our team. As a Housing Support Worker, you will be responsible for providing case management services to assist households in crisis to access and sustain suitable housing within our Housing Programs team.

Key Responsibilities:
  • Actively commit to and perform work in accordance with quality standards and continuous improvement;
  • Identify and communicate improvement opportunities, ensuring legislation compliance and regulations affecting the position are met;
  • Empower participants to obtain and sustain long-term housing outcomes by utilizing motivational interviewing techniques and trauma-informed practices;
  • Collaborate with internal and external stakeholders to achieve positive outcomes for families;
  • Conduct assessments of a person's situation and support needs, including risk assessments, to inform approach to all aspects of the work.
About You

The ideal candidate will possess critical thinking skills, remain highly organized, and be self-reflective. You should have experience working in a multi-disciplinary team and contributing to positive outcomes for families. Ability to develop networks and collaborative relationships with external providers is essential.

Requirements:
  • Diploma qualification in Community Service or a degree qualification with relevant experience;
  • 1 year experience in Social Work;
  • A current driver's license is essential;
  • Ability to work collaboratively with other staff in contributing to the development of work goals openly and honestly;
  • Experience working with people from diverse, social, and economic disadvantaged backgrounds;
  • Current National Police and Working with Children (Blue Card) check or the ability to acquire it.

This role contributes as an integral member of the Housing Services team, supporting all activities aligned with the strategic direction and consistent with the mission and values of Churches of Christ. The location is based across Brisbane, Logan, and Ipswich regions.
