**Job Summary:**

We're seeking a highly skilled and dedicated support worker to join our team. As a Disability Support Worker, you will play a vital role in providing exceptional support services to individuals with disabilities.

**Key Responsibilities:**

- Deliver high-quality support services that promote independence, dignity, and choice for our customers.

- Maintain effective communication and strong relationships with key stakeholders, including customers, families, and multidisciplinary teams.

- Develop and implement individualised support plans that address the unique needs and goals of each customer.

- Provide emotional support and companionship to customers, promoting their mental health and wellbeing.

- Work collaboratively with colleagues to achieve positive outcomes for our customers.

**Requirements:**

- Current Open Driver's Licence
- Full COVID 19 Vaccinations
- Current First Aid and CPR Certificate
