About the Role:

Cogent Thinking is seeking an experienced nurse or Allied Health professional to join our team as a Telehealth Care Coordinator. As a Telehealth Care Coordinator, you will play a vital role in delivering goal-directed, early intervention tele-health services to our clients.

Key Responsibilities:

  • Support and care for clients with empathy and expertise to improve their personal health and wellbeing goals.
  • Undertake coordination of supports and wellbeing services for a highly engaged and motivated client population.
  • Provide initial early intervention triage with clients, supporting both physical and mental health services.
  • Undertake relevant case coordination activity, including liaison with stakeholders, provision of email updates, and creation of appropriate documentation.

Requirements:

  • Experienced Nurse or Allied Health practitioner with full registration.
  • Mental Health support service experience highly desirable.
  • Caring and empathic nature.
  • Experience in care coordination or occupational rehabilitation.
  • Well-developed written and verbal communication skills.
  • Good IT and time management skills.
  • Strong customer service focus.
