About the Role

We are seeking a highly skilled and experienced Community Care Coordinator to join our team in Tamworth. As a Community Care Coordinator, you will play a vital role in building and supporting social, family, and community networks for our clients. You will be responsible for overseeing care plans, conducting assessments, and developing goals to assist clients in living safely and independently in our community.

Key Responsibilities
  • Deliver high-quality services in collaboration with the team
  • Liaise with other service providers to ensure seamless care delivery
  • Work with clients to develop community care programs, including Home Care Packages, Commonwealth Home Support Program, NDIS, and ICare
  • Manage client cases and develop Day Programs
  • Support staff in their roles to maintain high standards of care
Requirements
  • Strong leadership and people management experience
  • Business acumen to drive results through sales and marketing strategies
  • Previous experience in a similar role or within a similar industry
  • Qualifications and experience in Community Care Coordination or a Registered Nurse with AHPRA (not essential)
  • Current Class C driver's license
  • Proficient computer skills
  • Current NDIS Worker Check - NDISWC (or willing to obtain)
