Job Title:
">Care Delivery SpecialistThis role combines hands-on clinical care, staff education and service improvement. As a key member of our team you will lead the way in expert wound care management across both inpatient and outpatient settings.
You will be responsible for assessing, coordinating, educating and managing patients with acute and chronic wounds.
The ideal candidate will have passion for wound care, be able to work effectively in a fast-paced environment and possess strong communication skills.
Key Responsibilities:
- Assessing patients with acute and chronic wounds.
- Developing and implementing care plans to ensure effective wound healing.
- Coordinating care among multidisciplinary teams to optimize patient outcomes.
- Educating staff on best practices in wound care.
- Identifying opportunities for service improvement and implementing changes as needed.
Qualifications and Skills:
We are looking for a highly skilled and experienced nurse who has a strong background in wound care management. The ideal candidate will possess:
- A current registration as a RN/RPN.
- A minimum of 2 years of experience in a clinical setting.
- Specialized training in wound care management.
- Excellent communication and interpersonal skills.
- Able to work effectively in a fast-paced environment.
