Job Summary

We are seeking a skilled CDM Nurse to join our team at ForHealth Group (Dayforce) as a Chronic Disease Management Specialist. As a key member of our healthcare team, you will play a vital role in supporting our patients in managing their chronic conditions and promoting preventative healthcare.

About the Role

This is an exciting opportunity for a motivated and experienced Chronic Disease Management Nurse to join our team and contribute to the delivery of high-quality patient care. As a CDM Nurse, you will work closely with our General Practitioners and patients to identify and manage chronic conditions, and provide education and support to empower patients to take control of their health.

Key Responsibilities

  • Develop and implement comprehensive care plans to support patients with chronic conditions
  • Conduct health assessments and provide education to patients on disease management and prevention
  • Collaborate with General Practitioners and other healthcare professionals to ensure seamless patient care
  • Provide coaching and support to patients to promote healthy behaviors and lifestyle changes
  • Manage administrative tasks related to CDM appointments and patient care

Requirements

  • AHPRA registered Enrolled or Registered Nurse
  • Proven experience in Chronic Disease Management and health assessments
  • Strong organizational and time management skills
  • Excellent communication and customer service skills
  • A passion for delivering high-quality patient care and promoting preventative healthcare
