About the Role

This is an exciting opportunity to join our Health Independence Program as a HIP Care Coordinator. The successful candidate will be responsible for triaging inpatients, including clients attending our emergency department, to assist with case management in partnership with their GP and other supports.

Key Responsibilities

  • Work with GPs and other supports to promote effective management of care needs">
  • Collaborate with the HARP team to manage individual clients and run group programs to support people with chronic disease">
  • Participate in rehabilitation programs to support client recovery">

Requirements

We are seeking a registered nurse with experience in cardiac or pulmonary nursing. The ideal candidate will have excellent interpersonal skills and the ability to work well as part of a team.

Salary and Benefits

The estimated annual salary for this role is $80,000-$120,000, depending on experience. You will also receive a comprehensive benefits package, including access to our Staff Wellbeing Centre and workplace wellbeing programs.
