Job Opportunity: Support Services for Vulnerable Families

The role of a Home Hub Provider is a unique opportunity to make a positive impact on the lives of vulnerable Australians.

This position involves providing support services to families participating in the Mockingbird Family Constellation, a program aimed at promoting healthy communities and supporting foster and kinship carers.

As a key member of the team, you will be responsible for developing and maintaining relationships with foster and kinship carers, enabling them to provide a stable and supportive environment for children and young people.

This includes:

  • Coordinating and hosting monthly Constellation meetings to facilitate communication and collaboration among team members
  • Hosting meetings at the home when possible, such as case planning meetings, care team meetings, sibling and/or family visits
  • Maintaining regular contact with satellite families and children/young people to ensure their needs are met and they receive appropriate support
  • Providing help to satellite families in developing plans for children's success in school, identifying mental health resources, and accessing other community-based services
Key Responsibilities
  1. Coordinate and host monthly Constellation meetings to promote effective communication and collaboration
  2. Host meetings at the home when possible to facilitate face-to-face interaction and build relationships
  3. Maintain regular contact with satellite families and children/young people to ensure their needs are met and they receive appropriate support
  4. Provide support to foster and kinship carers to enhance their ability to provide a stable and supportive environment for children and young people

To be successful in this role, you must possess significant experience as a foster/kinship carer (5 years or more preferred) or employed experience in the Child Protection or Out of Home Care sector. You must also have a current First Aid Certificate and a current Driver's licence.

A high level of emotional energy, maturity, skills, and experience are required to provide support to foster carers, children, and young people in the Constellation.
