Care New England is seeking an experienced internal medicine specialist to join our innovative Hospital at Home team. As a physician on our HaH program, you will have the unique opportunity to deliver high-quality hospital-level care to patients in the comfort of their own homes.
Our Hospital at Home program has achieved impressive results, including improved patient outcomes, reduced complications, and enhanced patient satisfaction. This care delivery model is part of a growing national trend and has been shown to reduce hospital readmission rates.
As a member of our team, you will be responsible for managing acute inpatient conditions, conducting home visits, and facilitating admissions to the program. You will also work closely with our multidisciplinary team to provide comprehensive care to our patients.

Key Qualifications:
