Job Title:

We are seeking an Advanced Care Practitioner to join our healthcare team.

Key Responsibilities:
  • Undertake clinical duties under the direction of the Clinical Lead, including client assessments, medication management, and care planning.
  • Liaise with stakeholders to ensure appropriate care for clients.
  • Collaborate with the Operations Team to achieve strategic objectives and promote a continuous improvement program.
  • Act as a role model and mentor for colleagues.
  • Develop and review new and existing protocols.
  • Provide relevant training to staff and deliver education to frontline workers.
  • Attend to participants/clients to deliver specific clinical care/interventions.
Qualifications:
  • AHPRA registered nurse with no restrictions.
  • Postgraduate qualifications in a relevant field and minimum 2 years' experience in a clinical role.
  • Experience in community/nursing settings is preferred.
  • Strong focus on person-centered care and quality client outcomes.
