As our Care Manager, you will be responsible for overseeing the daily clinical operations of the care home, supporting and leading the care home clinical team, and ensuring the highest standard of ongoing assessment, care planning, and evaluation.
Key Responsibilities- Oversee the daily clinical operations of the care home, ensuring the highest standard of care is delivered to residents.
- Support and lead the care home clinical team, including coaching, mentoring, staff allocations, and delegation.
- Review clinical incidents, complaints, and changes, and lead compliance with ACFI assessments.
- Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
- Prepare and conduct clinical governance audits, and oversee medication management in accordance with resident needs and legislative requirements.
- Liaise with allied health, medical practitioners, and other services to provide optimal resident care.
- Current AHPRA registration is essential.
- Bachelor of Nursing or Equivalent is essential.
- Post-graduate qualification in aged care is highly regarded.
- Experience managing a minimum of 30 beds is required.
- Proven track record in delivering quality clinical care within an aged care or acute setting is essential.
- Excellent clinical assessment skills and care planning capabilities are required.
- Highly effective communication, leadership, and relationship management skills are necessary.
- An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes is essential.
