We're seeking someone who excels at guiding older Australians towards great care outcomes, loves solving problems and is great with people.

This isn't traditional aged care. We're building something better, and we'd love your help to deliver high-quality support services that empower clients to make their own choices, their way.

  • Provide case management and support via phone and email to a dedicated portfolio of clients, helping them navigate complex issues and sensitive situations.
  • Partner with clients to create, review and adjust personalised care plans that cater to their unique needs and goals.
  • Build trusted relationships with clients, families and providers by navigating complex issues and sensitive situations, ensuring seamless collaboration and communication.
  • Confidently handle challenging conversations to improve client independence and quality of life, leveraging proactive problem-solving skills to identify risks and opportunities.
  • Contribute to a positive team culture through shared learning, collaboration and open communication, driving excellence in every aspect of our work.
Key Requirements:
  • Customer service or call centre experience (case management, client support or account management highly regarded)
  • Strong communication skills to educate and guide clients through complex conversations, leveraging empathy and active listening to build trust and rapport.
  • A proactive approach to uncovering opportunities that benefit clients, identifying innovative solutions to drive improved outcomes and satisfaction.
  • Calm, solutions-focused mindset when challenges arise, with the ability to quickly learn and adapt to new technology and digital tools.

  • Experience working with older adults, either directly or indirectly, with a deep understanding of their unique needs and challenges.

  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families and colleagues.
  • Proven problem-solving skills, with a solutions-focused mindset and a willingness to take calculated risks to drive innovation and improvement.
