
Dedicated Homeless Support Specialist

TheHomes First Case Manager will have a clear understanding of the Housing First principles and assist individuals or families to obtain and maintain safe and stable long-term housing. This will be achieved through thorough needs assessments and effective support planning for each individual family member (including accompanying children), providing practical support where necessary, and connecting individuals and family members with the Homes First multidisciplinary teamor with appropriate additional support services.
This role is dedicated to helping clients achieve their housing goals and address their mental and physical health needs. The ultimate aim is to secure suitable and sustainable housing while supporting clients' recovery from personal challenges, thereby working to prevent future episodes of homelessness.

- Maintaining accurate records and reporting progress to stakeholders as required">
For success in this role, you will possess excellent communication skills, a strong understanding of housing first principles, and the ability to work collaboratively with diverse stakeholders. You will also be able to think critically, solve problems creatively, and provide empathetic support to individuals and families experiencing homelessness.
