Enhance Transition Outcomes for Individuals with Mental Health Needs
We are seeking an experienced professional to work closely with individuals who require support transitioning back into the community from a rehabilitation centre.
This is a fixed term, full-time opportunity until June 2027.
About the Role:
The Assessment and Transition Coordinator plays a vital role in facilitating the successful reintegration of prisoners with mental health needs.
Your key responsibilities will include:
- Delivering tailored transition programs to meet the complex needs of individuals with serious and significant psychiatric conditions, as well as those with stable psychiatric conditions
- Conducting comprehensive risk assessments and needs assessments, and contributing to the Case Management Review process
- Collaborating effectively with local service providers to ensure seamless support for individuals upon release
About You:
To be successful in this role you will need to have:
- Proven understanding of the specific transition and reintegration issues facing individuals with mental health conditions
- Excellent communication skills, with the ability to engage sensitively and effectively with diverse communities
- Capacity to work collaboratively with various stakeholders in a dynamic environment
