**Job Opportunity:** We are seeking a skilled Clinical Pharmacist to join our multidisciplinary team in primary care. This is an exciting opportunity for a motivated pharmacist to take on new challenges and contribute to the delivery of high-quality patient care.


The successful candidate will be responsible for providing clinical pharmacy services to patients, working closely with healthcare professionals to assess, review and manage medication use. Key responsibilities include:

  • Clinical assessment and review of patients to ensure safe and effective medication management
  • Polypharmacy medication reviews with patients to identify opportunities for improvement
  • Medicines-related queries answered and resolved promptly
  • Assessment of patients' ability to manage their medications, including home visits when necessary
  • Medicine reconciliation and prescribing of medication as required
