Job Description

This role will support you to work to the top of your clinical scope, providing quality care across a variety of work settings and caseloads.

You will be responsible for delivering evidence-based assessments, recommendations, and active interventions for a range of programs in the comfort of our customers' homes.

Your key responsibilities will include:

  • Delivering hospital substitution programs (Rehab at Home, Hospital in the home) - Acute and sub-acute rehabilitation in the home.
  • Government Aged Care programs (Home Care Package, Short Term Restorative Care, Commonwealth Home Support Programs) - Supporting our ageing population to stay well and independent at home.

You will work independently in the community and collaborate closely with our multi-disciplinary team for optimal patient outcomes.

You will engage in our clinical supervision program with a designated supervisor.

Requirements
