Key Responsibilities- Deliver holistic nursing care to clients, including managing a caseload of home care package clients and ensuring their needs are individually assessed and met in line with McLean Care's values and policies.
- Use clinical knowledge and experience to provide leadership, support, and guidance to other team members.
- Develop and implement care plans that meet the needs and goals of clients, within their budget and in collaboration with other healthcare professionals.
- Provide exceptional communication and interpersonal skills to build strong relationships with clients, families, and other stakeholders.
- Work collaboratively with the Community Program Manager to achieve team goals and objectives.
- Comprehensive clinical knowledge, particularly regarding age-related health issues.
- Previous care planning and clinical assessment experience.
- Engaging leadership skills and the ability to support staff to deliver quality outcomes for clients.
- Strong communication, interpersonal, and negotiation skills.
- Ability to assist clients to formulate a care plan within their budget and manage client expectations empathically.
- Current APHRA Registration as a Registered Nurse or Enrolled Nurse.
- Current First Aid and CPR certifications.
- An unrestricted driver's license.
- Knowledge and understanding of Consumer Directed Care packages will be highly regarded.
