About the Role

We are seeking a dedicated Care Coordinator to join our Community Living Team in Western Australia.

As a key member of our team, you will play a vital role in ensuring we deliver high-quality support services to individuals with Autism.

Your primary responsibility will be to lead a team of Support Workers, fostering a positive and inclusive environment that promotes the dignity and worth of each individual.

You will work closely with families, community stakeholders, and the people we support to create opportunities for them to achieve their goals and live fulfilling lives.

To succeed in this role, you will possess excellent leadership skills, a deep understanding of client needs, and a proven track record of effective service delivery.

In addition, you will be an experienced communicator, able to engage effectively with individuals from diverse backgrounds and build strong relationships with clients, families, and other stakeholders.

The successful candidate will be a strong advocate for the rights of people with disability, passionate about creating opportunities for individuals to have a good life.

Essential Requirements
  • Certificate, Diploma or Degree in the Human Services field.
  • Extensive practical experience in the field of Human Services, demonstrating a deep understanding of client needs and a proven track record of effective service delivery.
  • Experience in leading teams and managing people, overseeing day-to-day operations and client care standards.
  • Proficient in communicating effectively with individuals from diverse backgrounds.
  • Experienced in engaging with and managing relationships with clients, families, and other stakeholders, ensuring collaborative partnerships and effective service coordination.
  • Prior hands-on experience in implementing personalised support plans for individuals with disabilities, emphasising creativity, flexibility, and a person-centred approach to enhance quality of life and independence.
  • Possess a valid and unrestricted driver's licence (No P Plates)
