Job Opportunity:

This role involves providing clinical and holistic care to individuals in their homes.

Duties and Responsibilities:
  • Conduct assessments of clients in their residences
  • Ensure follow-up actions are taken to mitigate risks, refer allied health professionals, order necessary equipment and maintain documentation
  • Ongoing case management supporting clients' clinical and holistic needs
  • Monitor and communicate on the client's wellbeing and report progress, including care assessments, care plans and healthcare plans
  • Document clients' goals, progress and outcomes using person-centered, wellness, reablement and ablement strategies
  • Perform administrative tasks such as data entry, processing invoices, following up client inquiries and ensuring documentation for compliance purposes
  • Gather and upload evidence, including quotes, confirmation of delivery of products and services
  • Maintain accurate documentation of all evidence requirements in line with compliance regulations
  • Oversee and manage clients' budgets (government funding)
  • Collaborate with medical practitioners and allied health professionals as required
  • Ensure services are delivered in a caring and respectful manner, adhering to policy and procedures, legislative requirements and program-specific guidelines
  • Provide training, coaching and guidance to support staff, enrolled nurses and community support professionals as needed
  • Conduct skills assessments of community support professionals performing complex care services when required
  • Assist in scheduling client services, including service planning, confirmation and short notice placements or cancellations
  • Liaise with aged care assessment teams, My Aged Care portals, PRODA and other regulatory bodies
  • Demonstrate commitment to best practice and evidence of ongoing professional development
  • Participate in staff meetings and attend training opportunities when necessary
