Person-Centred Support Specialist

We are seeking a passionate and dedicated Person-Centred Support Specialist to deliver holistic coordinated support to clients engaged in the WARATAH Program.

  • Deliver comprehensive care plans that enhance health and well-being by connecting clients to vital health and social services.
  • Craft personalised support strategies that re-establish connections to community groups and essential services.
  • Provide tailored guidance to clients, their families, and caregivers to boost health literacy and promote healthy lifestyle choices.
Key Responsibilities:

  2. Support Planning: Craft personalised support strategies that enhance health and well-being.
  3. Guidance and Education: Provide tailored guidance to clients, their families, and caregivers.

Requirements:
  • Qualifications: Relevant tertiary qualifications in nursing, Aboriginal health, allied health, community/human services, social work/social science or equivalent.
  • Understanding: A solid understanding of the broader determinants of health including social, economic, and environmental factors and their impact on communities.
  • Experience: Proven experience in co-developing person-centred support plans that prioritise client wellbeing and aspirations.
  • Advocacy: Advocacy skills to enhance client access to programs and inspire community participation.
  • Communication: Exceptional communication and interpersonal abilities particularly with culturally and linguistically diverse backgrounds and First Nations communities.
