Description of the RoleThe role involves working with Aboriginal people to empower them to take control of their health and wellbeing needs.
Services are provided in various locations through main services and regional outreach programs.
This role offers direct patient contact and clinical support in Maternal and Child Health and Chronic Disease Management.
- Liaising between clients and clinic staff is essential for effective communication.
- Providing referral services and monitoring local health problems helps ensure patients receive optimal care.
- Participating in health promotional activities and preventive care tasks contributes to improving overall health outcomes.
- Collecting and coordinating health statistics provides valuable data for future planning and decision-making.
Some key responsibilities include:
- Developing and implementing healthcare programs for Aboriginal communities.
- Collaborating with multidisciplinary teams to deliver comprehensive healthcare services.
- Conducting health assessments and developing individualized plans to address specific health needs.
- Engaging with community members to promote healthy lifestyles and disease prevention.
The ideal candidate will possess:
- A Certificate III or IV in Aboriginal and/or Torres Strait Islander Primary Health Care.
- Experience working independently and as part of a team.
- Excellent communication, time management, and organisational skills.
- Computer literacy and report writing capabilities.
- Knowledge of specialist medical and allied health services.
