Job Summary:

We are seeking a highly motivated and caring Therapy Assistant to join our Home and Community Care team in the South Metropolitan region. As a key member of our team, you will play a vital role in delivering high-quality therapy services to our clients, enabling them to maintain and improve their independence and function.

About the Role:

The successful candidate will work under the guidance of our experienced Physiotherapists and Occupational Therapists to deliver individualized intervention programs tailored to meet the unique needs of each client. You will assist clients with group and individual programs designed to maintain and/or improve independence and function, working collaboratively with our therapy team to achieve optimal outcomes.

Key Responsibilities:

  • Deliver programs at Day Therapy Centres and clients' homes with a reablement approach
  • Contribute to the evaluation of therapy care plans
  • Assist clients in maintaining/improving functional independence
  • Complete documentation contemporaneously
  • Provide feedback to therapists on the outcome of interventions
  • Collaborative involvement in the Transition Care Program by delivering tailored goal-focused interventions

Requirements:

  • Certificate III in Allied Health equivalent or higher qualification - essential
  • Experience working as a Therapy Assistant - desirable
  • Experience working in Aged Care - desirable
  • Strong person-centered skills and ability to engage with clients of all abilities
  • Planning and organizational skills
  • Competency providing a range of therapeutic interventions/activities to clients individually and in a group
  • Current Australian driver's license and reliable vehicle - both essential
