About the Role:
We are seeking a full-time Clinical Care Coordinator to join our team in Adelaide CBD. The successful candidate will be responsible for coordinating clinical services to support clients in their own home.
As the primary point of contact, you will work closely with clients and staff to ensure efficient service delivery and provide the best possible outcomes.
- Daily coordination of care for new and existing clients of the SA Health Community Care Program.
- Acceptance of referrals including the Metropolitan Referral Unit (MRU) and ensuring all client information is available to commence service delivery within the required timeframes.
- Identifying clients in the clinical programs that are eligible for other Government funding streams and liaising with the referrer, MRU or healthcare professional for transfer of care.
About You:
To be successful in this role, you will have a Bachelor of Nursing, experience in Acute Nursing, Community nursing experience, and a Full Driver's Licence. Previous experience managing service contracts and staff is also highly desirable.
You will possess strong problem-solving and time management skills, effective communication skills, and computer skills. Ability to work over a 7-day roster and logistical experience are also essential.
