This role involves delivering high-quality, person-centred care to participants ensuring that services are consistent with the relevant legislation and funding guidelines.
- Implementing and maintaining a person-centred care model to enhance individuals' wellness and engagement.
- Ensuring compliance with legislative requirements and upholding Aged Care Quality Standards.
- Managing and overseeing a portfolio of community-based Aged Care Participants.
- Coordinating and overseeing the implementation of care plans.
- Creating and maintaining accurate Participant records, documentation, budgets and progress notes.
- Delivering high-quality, person-centred care to participants.
- Managing and overseeing a portfolio of community-based Aged Care Participants.

- Collaborating with other teams to ensure participant needs are met.
- Cert IV or Diploma in Community Services/Case Management (desirable)
- Relevant qualifications or equivalent experience in Aged Care Case Management
- Strong administrative skills and the ability to build rapport with staff and Participants.
- Demonstrated ability to work co-operatively as a member of a professional team.
- Proficient in the use of computer software including word, excel, internet and email.
