We are seeking a highly skilled and compassionate Senior Case Coordinator to join our team at Northern Adelaide Rehabilitation Service. In this role, you will play a pivotal part in ensuring seamless coordination and delivery of high-quality services under the National Disability Insurance Scheme (NDIS).

As a Senior Case Coordinator, your primary responsibilities will include conducting comprehensive assessments, developing tailored support plans, and facilitating effective communication between clients, families, and service providers.

The ideal candidate will possess substantial experience in case management and disability services, with exceptional communication, problem-solving, and organisational skills.

Key Requirements:

  • Appropriate degree or equivalent qualification for one of the following Allied Health disciplines: Occupational Therapy, Physiotherapy, Speech Pathology, Dietetics, Social Work, Podiatry, Clinical Psychology, or Exercise Science.

In this role, you will be an expert within your area and stream of practice and have a significant degree of autonomy and decision-making.

You will be part of a supportive team delivering high-quality care to the community, with opportunities for professional growth and development.

Deliver exceptional client-centred services

Conduct comprehensive assessments and develop tailored support plans

Facilitate effective communication between clients, families, and service providers

Work collaboratively as part of a multidisciplinary team

Ensure seamless coordination and delivery of high-quality services under the NDIS

Utilise your expertise and experience to drive positive outcomes for clients

Stay up-to-date with relevant policies, procedures, and best practices

Salary: $114,314 - $124,449 p.a.
