About the Role

An opportunity exists to join St Vincent's Hospital Melbourne as an experienced allied health professional/nurse in the Health Independence Program (HIP) Central Intake team.

As a HIP Intake Clinician, you will be responsible for managing referrals, identifying client needs, and accepting referrals for the most appropriate HIP service/s. You will work closely with various services to ensure accurate knowledge of eligibility criteria and current wait times.

Main Responsibilities
  • Triage referrals considering clinical need, risk, urgency, and service capacity.
  • Conduct initial needs identification and prioritisation of new referrals according to HIP Guidelines.
  • Advocate on behalf of clients as necessary and make referrals to other agencies when required.
  • Provide education, support, and advice to referrers regarding the HIP suite of service provision, client eligibility, and alternative referral options.
  • Offer allied health/nursing advice and support to the wider HIP Central team.

The successful candidate will possess strong problem-solving skills, excellent clinical management and communication skills, and a working knowledge of the healthcare system. They will also be able to work within a high-demand, dynamic environment and prioritize competing demands effectively.
