Our team is seeking a highly skilled Mental Health Clinician to join our Assertive Outreach Team. This role involves working with individuals who require specialized support and guidance in managing their mental health.The ideal candidate will possess excellent communication skills, a compassionate approach, and the ability to work effectively in a fast-paced environment.We are looking for someone who is passionate about providing evidence-based care, promoting recovery, and coordinating care plans that meet the unique needs of each individual.This position requires the ability to work collaboratively as part of a multi-disciplinary team, participate in quality improvement activities, and contribute to administrative responsibilities as needed.The successful candidate will have• A qualification recognized for registration with the relevant health board and demonstrated experience as a Mental Health Clinician.
• Proven extensive clinical experience in community mental health services and evidence of effective planning, implementation, and evaluation of care plans for complex mental health consumers.
• Experience in writing NDIS reports, mental health review tribunal reports, and effective care planning, implementation, and evaluation of consumer goals.
• A current driver's license.
