
Empower Participant Outcomes as a Support Coordinator

Join our vibrant Mental Health team in Sydney as a casual Support Coordinator and play a crucial part in helping NDIS participants connect with and maintain their funded services. Enjoy the flexibility of working remotely, with the option to use our office as needed.
In this role, you will:
- Ensure best possible outcomes for participants are achieved by supporting participants with implementing their NDIS plan and identifying and connecting with appropriate services that enable participants to work towards and achieve their goals.
- Ensure choice and control of participants and their safety and welfare drives all actions and decision making by working collaboratively with service providers, carers and other organisations.
- Facilitate and encourage participants to increase their capacity to become more independent with managing their plans.
- Maintain a full participant caseload by working within organisational guidelines and maintaining designated daily tasks.
Key Responsibilities include ensuring timely completion of administration tasks and maintaining files to ensure compliance, as well as representing the NDIS Mental Health team and engaging in promotional activities to generate referrals.
About You:
As a successful candidate, you should have relevant tertiary qualifications or be studying towards which may include Social Welfare, Mental Health, Social Work. A good understanding of the National Disability Insurance Scheme is also essential. Experience in working with people with complex mental health challenges is highly valued, as is a valid Working with Children's Check for paid employment purposes. Excellent written and verbal communication skills are also required, along with experience in providing support coordination under the NDIS if possible.
