Job Description

The role of Support Coordinator is a permanent full-time position requiring the ability to work 38 hours per week, Monday to Friday between 8am - 5pm.

This involves implementing participant plans to assist individuals living with disability in achieving their personal goals and developing necessary skills.

Key responsibilities include:

  • Providing support coordination, specialist support coordination, and psychosocial recovery coaching to NDIS participants
  • Collaborating and working with participants, family, carers, and other services
  • Liaising with stakeholders, community services, government agencies, and other service providers
  • Preparing and maintaining up-to-date and accurate case management notes and reports
  • Ensuring the full implementation of the participant's NDIS plan and identifying, developing, planning, designing, and coordinating NDIS supports to assist the participant to meet their NDIS and personal goals
Required Skills and Qualifications

Applicants should have:

  • Experience working with people with disability
  • Experience working with people with psychosocial disability
  • Good organisational skills and excellent communication, interpersonal, and time management skills
  • A valid Blue card and Disability Worker Screening Clearance (Yellow Card)
  • A Queensland Drivers Open Licence and ability to use own car
  • Completed NDIS Worker Orientation Module, First Aid & CPR Training, and Covid-19 digital certificate
